PCSD Statewide Standardized Assessment Opt in Form

The Putnam County School District makes statewide standardized assessments (outlined in s.1008.22(3) F.S.) available to students in Putnam County who participate in home education, scholarship programs, and other choice options. The available statewide assessments are only for students in grades 3-12. K-2 assessments are part of Florida's coordinated screening and progress monitoring system and are not available.  

Completing the form serves as official notification of a student's voluntary intent to participate.

The dates and times for Spring 2026 state testing are not yet established for students "opting in to state testing."  Check back on March 1, 2026 when the form opens for exact dates and times for the following tests:

  • Writing  Grades 4-10  (120 min.) 

  • FAST Reading Grades 3-10 (120 min.)

  • FAST Mathematics (Grades 3-5, 100 min.) (Grades 6-8,  120 min.) 

  • Algebra EOC (160 min.)

  • Geometry EOC (160 min.)

  • US History EOC (160 min.) 

  • Science- Grades 5 & 8 Only (160 min.)

  • Biology  (160 min.)

State assessments will be administered at C.L. Overturf District Center, which is located at 1100 S. 18th Street, Palatka, FL 32177.

If you cannot participate on the designated days, or your child is K-2, please refer to the list of alternate Norm-Referenced Assessments on the Florida Tax Credit Scholarships page on the FLDOE website.

Students will need one of the following types of photo ID on the day(s) of testing:

  1. State or Federal-issued photo ID for my child: Florida-issued photo ID can be purchased at the local branch of the Department of Motor Vehicles 

  2. PCSD Photo ID form prepared by PCSD. You will need to upload a digital color photo of your child (headshot). The PCSD Photo ID forms will be kept at the testing site and you will sign in with the form each test day to verify identity.

If your child has an IEP, LEP or 504 plan with testing accommodations, you may upload the digital version with the opt in form if you wish for testing accommodations to be provided to your child. You can acquire a copy from your child's last public school of enrollment.
