Controlled Open Enrollment Application
Applications must be submitted at the requested school site for the following school year between March 1 and July 1. Decisions on transfer requests are determined based on class size capacity limitations and may not be available until after the first ten days of school. Students attending through approved controlled open enrollment waivers are permitted to attend the school of assignment from year to year. New applications will only be needed when progressing from one school to another (ex. elementary to middle school). Students must remain in their zoned school pending the decision.

Hardship late filed applications must be based on a situation or circumstance that will have a compelling and adverse state of misfortune for that student or their family’s daily life.

Streamline the registration process by printing and completing the linked forms. Take them with you to register on campus.
Families currently experiencing certain living situations are eligible for certain federal rights that may help obtain registration documents and support immediate enrollment in school. View the McKinney-Vento Education Act to see if you are eligible for these services.
Some of these forms need to be notarized. The schools have notaries on-site for your convenience.
You will also need to bring the following items:
Parent or guardian picture ID
The student's Birth Certificate
The student's Social Security Card (Optional)
The student's Florida shot records (available from the Health Department at 2801 Kennedy St. Palatka FL 32177)
Proof of residency (light bill, phone bill, leases, etc)
If you are not the parent of the student you are registering, you must show legal proof of guardianship.
Student entering Pre-K or kindergarten will also need a Florida physical that is less than one year old from the first day of the new school year.
