As you prepare for the new school year, please ensure your child's health records and required forms are up to date before the first day of school.
Immunizations & Physicals: Florida school immunization requirements and physical examination requirements must be met for enrollment and schedule pick-up. Vaccinations are available through your child's healthcare provider or the Putnam County Health Department.
Medication at School: If your student requires prescription medication or medical treatments during the school day, a current physician-signed authorization must be on file before services can be provided.
Over-the-Counter Medication: Parent authorization is required before over-the-counter medications can be administered at school.
