Immunization Requirements
As you prepare for the new school year, please make sure your child’s required
immunizations and physical examination are up to date. Florida school immunization
requirements must be met for enrollment and schedule pick-up. Vaccinations are
available through your child’s healthcare provider or the Putnam County Health
Department.
If your student will need prescription medication or medical treatments during the school
day, a current physician-signed authorization must be on file before services can be
provided. Over-the-counter medications also require a signed parent consent form.
